Proven assistance to elderly, their families

If you are caregiver to your parent or parents, you typically still work, have children of your own to care for, a husband, and basic everyday responsibilities. There is not enough time in the day to complete your growing “to do list”. Then a monkey ranch is thrown into the daily routine if your mom or dad has to be admitted into the hospital.

Where do you go to seek support, get questions answered, figure out next steps, and what truly is best for your mom or dad?

This blog post is inspired by a recent article I read, not to mention, being a Care Manager for some time. This article described the everyday caregiver I encounter. However, working for a hospital system, I am limited in my abilities, this is the reason why I decided to become a Certified Geriatric Care Manager/Aging Life Care Professional® and own my own Aging Life Care™ Practice, (Evolve Senior Solutions, LLC). As an Aging Life Care Professional®  I can care for senior the way I love, the way they need to be cared for, holistically, and support caregivers along the journey.

I am going to take this opportunity to answer the questions discussed in the article:

‒ What are the primary services provided?
As a Registered Nurse, I provide a complete nursing, head to toe, assessment, each time, I see my patients. I also provided Medication Management, each time, I see my patients. In regards to the Geriatric Care Manager services, I provide services that address the needs of the patient in all areas of their life to include support to the caregiver. ‒ Is there a fee for the initial consultation? If so, how much?
No, I offer a free, initial call or I like to call it, a conversation, to determine what the issues, concerns, and needs are; then we arrange for a face to face consultation at the senior’s home. The face to face is done at the senior’s home because I do an environment and safety assessment as well, that will promote fall reduction and optimal independent living.
Since all face to face visits include services of both Register Nurse care and Certified Geriatric Care Management, the fee is by the hour, which is $80/hour.

‒ How many care managers are in the business?
I am the sole founder, owner, and operator. I accept a minimal number of clients because quality, not quantity is important to me.

‒ What are their professional credentials?
I am a Registered Nurse. I am a Certified Dementia Practitioner, Dementia Specialists, and Dementia Instructor. I am CPR certified as well as Mental Health First Aid.

‒ Are they licensed in their profession? If they are, ask them to provide a copy of their certification or look them up through the state to make sure their license is current.
Yes, I am a Registered Nurse. 

‒ How long have they been providing care-management services?
I have been in the specialty of care management for the past 5 years, I have worked in the community, insurance, and hospital settings.

‒ Can they provide references?
I can provide both professional and personal references.
